Community support program

Adult residents of Hennepin County with a diagnosis of a severe and persistent mental illness may join Community support programs. Staff help people: 

• Find and maintain competitive employment.
• Handle basic activities of daily living.
• Participate in social activities.
• Set goals and plans.
• Get and maintain appropriate living arrangements.

Join a community support program

• No referral needed. Go on a day the drop-in location is open.  
• Bring a copy of a diagnostic assessment stating your diagnosis. Or have your login information so you can access it online.
