Prepare application
- Complete a certificate of filing form (PDF).
- Print but do not sign the application.
- Get your signature on the application notarized (e-notarization is acceptable).
- Get a readable photocopy of your credentials of license or ordination; or a statement of marriage authority from a spiritual authority. (Do not send originals. Submitted credentials will not be returned.)
- Gather your application, copies of documents and a $30 check or money order made out to Hennepin County Treasurer.
