Long-Term Care Infection Prevention Program
Hennepin County Public Health created the Long-Term Care Infection Prevention Program to increase support for long-term care facilities’ infection prevention and control programs.
We do this by conducting program assessments for long-term care facilities and coordinating the Long-Term Care Infection Prevention Coalition of Hennepin County.
Assessments
Our team offers on-site and remote program assessments to long-term care facilities in Hennepin County. Assessments include Infection Control Assessment & Response (ICAR) or TeleICAR. These are free, non-regulatory assessments to help:
- Identify program strengths and opportunities for improvement
- Provide tailored resources
- Increase staff confidence to implement evidence-based practices

Coalition
We brought together a group of stakeholders to create a coalition. The Long-Term Care Infection Prevention Coalition of Hennepin County was the outcome. The coalition works to prevent health care associated infections and antimicrobial resistance.
The coalitions' mission is to promote a culture of infection prevention and control as everyone’s responsibility. There are several committees who work together to fulfill that mission.
Bringing together diverse perspectives, backgrounds, and knowledge increases our capacity to make change.
